Final Grade Report Request

The purpose of this form is to request an official final grade report to be mailed to your home address on record at Clarke. Requests for midterm reports will not be processed as midterm grades provide an indication of academic progress and are not permanent grades. All degree-seeking students can access midterm and final grades on-line through MyInfo.
