Incident Report
Report information is kept confidential by the University, to the extent allowed by law. No information regarding the sender (name, email, IP address, etc.) will be collected when submitting through this page. Report content goes to the Vice President of Student Life, Vice President of Academic Affairs, Director of Human Resources, and the Director of Intercultural Programs. It may be forwarded to appropriate officials to investigate, rectify, and recognize. Please be as informative as possible.
