Distinguished Alumni Nomination Form

Recognize a Clarke classmate by nominating them for the Distinguished Alumni Award to be given out at Homecoming. Awards include Outstanding Achievement, Humanitarian Service, and the Rising Star.

  • Outstanding Achievement Award is presented for outstanding professional achievement and success.
  • Humanitarian Service Award is presented for exceptional service to humanity through civic, faith-based, or educational endeavors.
  • Rising Star Award is presented for alumni 35 years of age and younger who have made an impact in their career, community, or contributions to Clarke.

All nominations must be submitted by Friday, March 19, 2021, at 4:30 p.m. (CST).

Please be as thorough as possible when filling out this form. Nominations are reviewed and winners selected by the Alumni Association Board. The 2021 award winners will be announced this spring.
